Challenges in Clinical Trials in Infection

There are numerous scientific and logistic challenges to the successful delivery of clinical trials of infections in high and low-and-middle income settings. However, for interventions to truly have global applicability, strategy studies of treatment optimisation for chronic infections which predominate in LMIC settings such as HIV and TB, must include subjects recruited in LMIC countries.

In this presentation I will overview the way in which a large international network, INSIGHT, delivered one of the most important treatment trials in HIV, the START study, across a large global network of sites in 5 continents. Next I will review some of the challenges of RCTs for acute infections in high and LMIC settings, and finally, how to foster and develop research capacity for infections research.
